If you have more than three accounts affected for one CTR form, are you required to list all of the accounts?
Answer: 

Yes, the CTR instructions for item 35 say:

Enter the account numbers of any accounts affected by the transaction(s) that are maintained at the financial institution conducting the transaction(s). If necessary, use additional sheets of paper to indicate all of the affected accounts.
