Reg E - Re-debit the Customer's Account?

We understand a Reg E dispute must have a decision made within 45 days of the customer notification. At the 45 day mark, the bank is still waiting on a merchant response so the bank makes a decision to finalize the dispute in the customer's favor since they can't keep it open or deny (no proof). The merchant (after the dispute has been finalized) then supplies documentation that proves the customer performed the transaction, can the bank contact the customer to advise and re-debit the customer's account?

Provisional Credit Predicament

Do we have to offer provisional credit within 10 days in situations where the card holder says in one claim they know nothing about it and in the next letter they say they cancelled the service? We inquired with the service provider and they insist the card holder has the equipment, the service, etc. and just needs to return everything. If we charge this back we will surely lose and the card holder will be long gone with our credit and we will not be able to reclaim it. Is there no provision in Reg E to account for participation in a transaction?
Website Changes & Website Record Retention

I have a question about website record retention. With regards to website changes (disclosures, marketing advertisements, general changes, etc.) is there a concrete retention schedule or best practices for retention? I see many companies treat these changes like a hard copy advertisement change. They will print out the archived page and save it for the examiner. If this is the case, is there a time frame to keep these archived print outs or do you just save them between examinations? I couldn't find any information on the FFIEC website that explains a policy and any website research is very vague.
