I.D. Theft Procedures and Customer Claim Form

BOL User Linda Kannenberg of First National Bank, Fort Collins, CO referenced her customer claim form with instructions in the BOL threads as well as her bank and fraud department procedures. She had so many requests to share that she has provided this as a Banker Tool.

These tools may be modified for your institution in the Word versions. They will provide guidance for the institution, the department tasked to handle the claim and even include a customer friendly form with detailed instructions to initiate the claim of identity theft.
