We are revamping our backroom EFT/Visa dispute process. Does anyone have a flowchart (or know of one) that we could have or purchase? The goal is to communicate decision points to staff in order to streamline processing and a visual would be really helpful.

A lot is going to be dependent on your internal procedures. But here's an example of what one could look like. I didn't really review it to see if there are any errors in it, but you could take your procedures and make one using this as an example. Here's another one from the merchant's point of view.
