Does anyone have a completed OFAC risk assessment that I can look at and use as a guide in creating one for my bank? Any help is greatly appreciated.

While some bankers are willing to turn over the sweat of their brow to others on request, the demand for such actions on BOL greatly exceeds the supply. You will get more consistent support from your peers when you are clearly working on the project yourself and make specific requests for help.

See Exhibit M of the new BSA/AML examination manual. Addressing each of the issues raised there; i.e. answering the questions based on the information you developed during your BSA/AML risk assessment, will not be difficult or time consuming.
