If we are limiting our OD program to one account type, and a customer wants to switch their account to another type (where they would no longer have access to the program), are we still required to print the aggregated NSF fees on their statement after the switch? Does 230.11(a)(4) apply in this situation?

No. The totals requirement only applies to the account types you've identified in your ads as having access to ODP. If you don't specify in those ads, however, it will apply to all deposit accounts of consumers.
