We have a HELOC applicant with a name discrepancy and would appreciate some advice.

Applicant's legal name on drivers license, deed, and 1st mortgage at other FI is Bernardine.

W2, Paystubs, Home Owner's Insurance, and bank statements are all Bernadine (missing the r).

Do you see any underwriting or compliance concerns if we have the Signature Affidavit and AKA statement signed by the member?

Additionally would you have the borrower sign all documents Bernardine AKA Bernadine? I suggesting simple titling all documents Bernardine (who can really tell on a signature if the "r" is included or not).

Thanks in advance.

This is more of a question for your legal counsel.

Really doesn't matter what any doc says. What does the customer say their name is? That's what you should use.
As for a/k/a, as explained to me by our attorney, a misspelling is not an a/k/a. An a/k/a is something like Bill when the person's legal name is William. Therefore, an explanation letter in the file stating which is a misspelling is better than trying to misuse an a/k/a statement.
