From 1991 to 1996 (I think '91, but don't hold me to it), bankers had to document the panel # and date of map used to make the flood determination. I remember this change when I worked for the FDIC. Some bankers didn't really look at the maps. Instead, they wrote down all of the panel #/dates on a piece of paper and referred to it when making a loan.
