We are a small community bank - our only "mortgage products" are a manufactured home loan product and a home equity loan product which we fund and retain servicing on. Thus, under the LO rule, the bank is a "creditor" and not a "loan originator". Am I correct then in that only our individual loan originator's (loan officer's) name and NMLSR unique identifier go on the covered loan documents? NOT the bank's NMLSR number? Or should we also be including our bank's number? Thanks!

I found it! As provided in 1026.36(a)(1), the term "loan originator" includes creditors that engage in loan originator activities. "Thus, for example, if an individual loan originator employed by a bank originates a loan, the names and NMLSR IDs of the individual and the bank must be included on covered loan documents." (page #538-539).
