Are there other requirements that should be on the short form loan application. I was not able to locate anything under Reg Z?

How about the Reg B intent to apply jointly language?

I see this is an older post, but the OP is looking in the wrong Regulation for application do's and don'ts. They should be referring to Regulation B. Asking for income without giving them the option to exclude certain types of income will get them in trouble also.
