Since the new settlement cost booklet is only available in PDF format, is it ok for the bank to have it printed up as a booklet ourselves?

Would it be alright to have the bank's name and phone number also added to it, for instance, on the back?

I have not seen any regulatory restrictions on this unless I missed them.

You should be fine doing that...and adding your name, etc is not uncommon.
