1) 604(b)(2)(A)(i), they are provided a document that consists solely of the disclosure, that a consumer report may be obtained for employment purposes
2) 604(b)(3)(A) ...in using a consumer report for employment purposes, before taking any adverse action based in whole or in part on the report, ...provide to the consumer ...a copy of the report; and a description in writing of the rights ...
3) Look to 609(c) The notice should be similar. (Although the FCRA refers to (c)(3), that doesn't exist in the FCRA, at least yet.)
