Customer presents payment to teller and asks to apply towards Commercial Line of Credit, payment is processed and pays off LOC and closes the LOC in error, customer only wanted to pay his line down. LOC is closed. Can we reverse this and re-open LOC?

Why would you not be able to reverse your mistake? Was collateral also released or something?
