Phone Stolen, Fraud Charges: Reg E?

A customer's phone was stolen, and because the debit card was on file with the phone company, the fraudulent charges were automatically applied to it. The customer is now wanting to file a dispute with us (the bank) to receive his money back ($200). How is this handled under Reg E and VISA? Should I file it as a VISA? Do I need to provide provisional credit, or does he need to take it up with his cell provider?
A-9 Needed for HSA Accounts?

Do we need the A-9 form for HSA accounts? Our HSA accounts are housed on the deposit side and debit cards are issued. However, I know that we do not charge overdraft fees on these accounts, so I am thinking that the A-9 is not necessary? Is that right?
Responsibility for commercial account debit errors

We are working on a project to make debit card access available to some of our commercial accounts, primarily to give them more flexibility with respect to purchases for their businesses. One of our questions concerns liability for debit card errors or fraudulent transactions. Should we simply "borrow" the provisions of Regulation E by reference in our documents, or should we be going in a different direction?
Authorized card user on decedent's account

We were just notified of the death of one of our debit cardholders. He passed away over the weekend. The person who told us about his passing is an authorized signer (not a joint owner) on our customer's account, and also has a debit card that accesses the account that she used to do shopping and other errands for our customer, who was often too ill to go out on his own. We know that we have to watch the account and stop paying any checks our customer drew once ten days have passed since his date of death, but what do we do about transactions conducted by the authorized signer, by check or by debit card?
Providing Documents on a Flash Drive

I work with commercial deposit and treasury management products. Would it be possible to provide new account documents (TandC), TM contracts, and signature card to the customer on a flash drive versus a paper copy?
