What regulations cover business accounts? Specifically, opening and management of accounts?

A customer has a DBA sole proprietorship using his social security number. Are there any regs, etc., that say it cannot be in a "personal type" checking account? We only have a "commercial account" and nothing for the small "mom and pop" type of business.
A customer came into the bank and wanted to purchase a cashier’s check. Instead of making his check payable to the bank, he insisted on making his check payable to Jane Doe (same payee name as would be on the cashier’s check). Our teller refused to do the transaction, because Jane Doe was not there to endorse the check in exchange for the cashier’s check. The customer said the bank is wrong and he wanted something in writing explaining why the bank refused to do this transaction. Are there any rules, Regs, laws or UCC codes for this particular issue?
In order for our customers to get the appropriate FDIC coverage for revocable trust accounts, how should an account be titled that has multiple beneficiaries, ssing the word "or", "and" or nothing at all? For example:1) Jane Smith ITF - John Smith, Dave Smith, Paul Smith2) Jane Smith ITF - John Smith, or Dave Smith or Paul Smith 3) Jane Smith ITF - Jane Smith and John Smith and Dave Smith Does it become the bank's responsibility to divide the estate equally between all beneficiaries upon a death?

Are there any Regs, laws, UCC codes, etc., that prohibit a blanket policy that a bank will not cash third party checks?
We want to advertise some home loan products on our bank statements. We can include "Equal Housing Lender", but there is no way to add the house logo. Since we can't add that, can we run the ad anyway?
Marketing feels as though statement stuffer after statement stuffer is not being read. They would like to add some of the messages to the outside of the envelope. It may be restated inside as well. Would this be a problem?
Are there any federal regulations that govern IOLTAs, IRETAs and funeral trusts? Do individual states have laws that govern these accounts? One of the departments we generally perform compliance testing on for escheat and court blocked accounts accepted responsibilities for these three new areas and we would like to confirm what additional compliance testing needs to be done.
